Who we will work with



We will work with the most difficult and vulnerable individuals who are often referred to as non compliant, difficult, not motivated to change and serial non attenders and/or are frequent attenders at hospital, who have complex mental health and substance misuse needs with multiple vulnerabilities , who are unable to access services in the traditional way.

What we do

Clients exhibit a range of needs and personal challenges, including individual, psychological and social engagement issues.

Our service design is based on an understanding of the interlinking of their individual needs and the complexities in addressing personal and social challenges when working towards improved health and well being, purposeful social interaction and an overall improved quality of life.

Referral Form and Criteria

Referral into the service will initially be through the criteria and a referral form that follows the criteria.

Clients must meet all areas in the criteria to be eligible for the service, If they do not meet all aspects they should in the first instance be referred to generic Substance Misuse Services such as CGL, SMD, Addiction SW Teams
